Employers:
Employers must fill out a group enrollment form, and mail or fax it in along with some basic company information.


Employees:
Once your company is signed up, your employees can use the Bright Choices Portal to learn about and enroll in the insurance benefits.

Login instructions for Bright Choices:

Group Identifier: Chautauqua
Username: First and last initials and the last 4 digits of your Social Security Number. (no dashes or spaces)
Password: Your full 9-digit Social Security Number (no dashes or spaces). The first time you log in, you will be asked to change your password.
